Microsoft has changed the way how add-ins work, the new model is described on MS website as a KB called" Outlook add-ins overview".
My customer wanted to add an admin called HERA (Hotel email reservation assistant) and it is not possible in KOFF profile only in native Exchange profile.
This new way is not compatible with Kerio KOFF nor Webmail, for some customers it is a deal breaker. Based on the investigation the new add-in uses MS Exchange API and expects a response from the server.

Hello Ian,
maybe I have expressed the feature request not in a clear way. The purpose of the feature request is not to rework KOFF to the new add-in model, I fully agree, that COM add ins will still be supported in a near future by Microsoft.
The request was initiated to make KOFF profile compatible with new 3rd party add ins to allow our customer to benefit from these new add ins. Currently they have to decide if to stay with us on Connect and not to use new add in, or (what is bad) to migrate to Microsoft email solution and use new add ins. It may result that we will loose some customers.
Best
Michal
