We have the same issue. Kerio Connect 9.0.4, clean install of 10.11.5. Configured account with Kerio Connect Assistant and Mail is complaining and re-creating "Delivered"-folder on each launch. Deleting Outbbox.mbox did not help.

We found a solution today:
Delete NVRAM of the mac was the key for ous. We cloned 16 iMac's with carbon copy cloner. We had the issue with the outbox on each staation except the original. After delete NVRAM an all other stations it works everywhere.

I only see this behavior if I use the auto-config tool.
An IMAP account, resident on Kerio or another IMAP server, configured by hand does not exhibit this behavior.
I don't see this behavior on accounts migrated to 10.11.x from earlier versions of macOS, either configured by hand or with the auto-config tool.
It would seem that something in the profile that is installed interacts poorly with Mail as it exists in 10.11.
It sems that none of the remedies proposed in this thread work 100% of the time on 100% of the systems out there. But there are enough systems having the problem that it should get some attention from Kerio so that it can be resolved in some fashion.

We have the same problem.
Kerio Connect 9.1.1
Clean installation of OS X 10.11.6 (Mail 9.3 3124)
Account created with Kerio Account Assistant.
I've been able to work around this by disabling Notes for the email account, but not this time. Outbox gets renamed every time Mail launches. None of the instructions on this thread have helped.
Adding the account manually works as expected.

I've also run into this problem. The workaround that has been so far successful for me is:
Make sure that the "Sent" folder contains at least one message before running the account assistant.
I now open webmail, compose and send one test message, and only then do I start the assistant. So far, the renamed folder conundrum has not appeared again.

This happened to me again several days ago on a client's in-house email server. They are running Connect 9.1.1. I created a new mailbox and ran the Kerio setup assistant. Launched mail and everything was OK. Quit Mail and re-launched it. Got the error about the Outbox getting re-named.
I then logged into Webmail and sent a message from there. I removed the profile from System Preferences (effectively removing the account) and re-ran the assistant. The problem persisted. I did not have a whole lot of time to devote to this so I just set up the account in Mail manually.

This has just happened to me for the first time.
I created a new user with a new home folder on OS X 10.11..6. The account is hosted on OSX Open Directory server and imported to Kerio Connect.
The Mail account was configured IMAP automatically by the Kerio profile tool.
The error message is
"Mail found an existing mailbox named Outbox; this mailbox is reserved for Mail. Your mailbox has been renamed to "Delivered X"."
X increments each time Mail is opened.
The new folder is created "On My Mac". However, when mail is sent t is stored in the Kerio 'sent' folder.
I have created many users and account without this problem occurring, so not sure what the difference is here.

Still the same problem.
Fresh install of 10.11.6
Kerio Connect 9.2
Set up accounts ussing the assistant. Everything works OK the first time you launch Mail. Quit, open again and the endless renaming of Outbox-folder begins.
This is extremely frustrating. The Kerio Connect Account Assistant was one of the best features in Kerio for a medium business and now it's unusable.
9.2.1 just came out and no mention of this bug.
Please fix this. And while you're at it, please also pay the $99 to Apple so that we don't have to do the control-click/open dance ("this application is from an untrusted developer") every time.
